Box 4 flows to the federal income tax withholding section of Form 1040 (combined with W-2 and other 1099 withholding into your total payments), not to Form 8949 or Schedule D, since it has nothing to do with the gain-or-loss calculation itself.
How Box 4 works
Box 4 reports backup withholding the broker was required to remit — typically because you had not furnished a valid taxpayer identification number on a Form W-9 (for U.S. persons) or the applicable Form W-8 (for foreign persons) before the transaction, or because the IRS notified the broker that your TIN was incorrect.
Backup withholding is a flat-rate prepayment of tax, similar in concept to withholding from a paycheck — it is not a separate tax on the transaction, and it is credited against your total tax liability for the year regardless of whether the underlying disposition ends up being a gain or a loss.
Because backup withholding usually signals a TIN mismatch on file with the broker, seeing an amount in Box 4 is worth investigating even after you have claimed the withholding credit — resolving the underlying W-9/W-8 issue prevents the same withholding from recurring on future transactions with that broker.
Worked example
Tom never submitted a completed Form W-9 to his exchange, so the exchange applies backup withholding on a $5,000 sale. Box 4 shows the withheld amount, which Tom claims as a federal tax payment on his Form 1040, crediting it against his total tax liability for the year regardless of whether the sale itself produced a gain or a loss.
