Medicare premium FAQs

Is Medicare Part A premium-free?

Part A is generally premium-free when the enrollee or a spouse has at least 40 quarters of Medicare-covered work. CMS charges a reduced premium for 30–39 quarters and the full premium for fewer than 30 quarters.

Does this calculator include IRMAA?

Yes. It adds the official Part B and Part D IRMAA surcharges for the selected premium year and MAGI lookback return. IRMAA is applied separately to each enrolled person.

Is the Part D national base premium my actual premium?

No. Actual Part D premiums vary by plan and location. Enter the premium for your chosen plan; the CMS national base beneficiary premium is mainly a formula input for plan pricing and late-enrollment penalties.

Are Medicare deductibles included in the annual premium result?

No. Deductibles and cost sharing depend on care received and are shown separately. The annual result covers Part A, Part B, Part D plan premiums, and applicable IRMAA only.
