A spouse who files separately but lived with their spouse at any time during the lookback year skips Tiers 1 through 3 entirely — MAGI above $112,000 would jump directly to the Tier 4 surcharge, if this projection holds.

Methodology — how this projection was built
Two different sources, because the dollar amounts and the MAGI thresholds are set by different processes. Dollar premiums and Part B surcharges: CMS's own actuarial projection under the intermediate set of assumptions — Tables V.E2 and V.E3 of the 2026 Medicare Trustees Report. Part D surcharges: derived from that report's projected Part D base beneficiary premium using the statutory formula in section 1860D-13(a)(7) of the Social Security Act, rounded to the nearest 10 cents, which reproduces every CMS-published year exactly. MAGI thresholds: a ustax.tools estimate, because no official figure can exist yet — 20 CFR 418.1105(c) sets them by applying the CPI increase to the prior year's thresholds, rounded to the nearest $1,000, and that CPI window does not close until August 2026. The estimate applies the actual 2025-to-2026 growth rate of the single-filer entry threshold (2.83%) to every 2026 boundary. Three parts of the ladder are law, not estimate: joint boundaries are exactly double the single boundaries below the top tier; the $500,000 / $750,000 top tier is fixed by statute and is not indexed until 2028; and the married-filing-separately top threshold is $500,000 minus the entry threshold, so it FALLS as the entry threshold rises.

If this projection holds, married-filing-separately filers who lived with their spouse during the lookback year would skip straight from Standard to Tier 4 once MAGI passes ~$112,000 — the same "no gradual climb" structure as every prior year. An MFS filer who lived apart from their spouse for the entire lookback year uses the regular single-filer table instead.
Is Medicare IRMAA 2027 a cliff or a gradual phase-in?
Still a hard cliff, regardless of the exact 2027 dollar thresholds — this structural feature of IRMAA is settled law, not something subject to projection. Crossing a boundary by even one dollar moves your ENTIRE MAGI into the next tier's full surcharge, not just the dollar over the line.
